Preventative Health Care Visits
Complete annual physicals are recommended annually for all ages. The purpose of these visits are to review a patient’s health history (medical, surgical, review of organ systems, family and social history) and to evaluate what diseases a patient is at risk for and what screening tests are appropriate for the patient. If a patient has a specific concern or physical complaint a sick
visit would be a more appropriate visit type to schedule and a
comprehensive physical can be scheduled at a later date.
Well Child Visit- evaluates child’s developmental (mental and physical) stages, administer appropriate vaccinations and order appropriate testing.
School Physical- required for day care, grade school or college admission. Please inform our office if we need to provide you with a “yellow” form (Florida State school physical form) or “blue” form (Florida State vaccination form) when you schedule your appointment. College admission forms should be brought in at the appointment time.
Sports Physical- required for high school or college athletics. Please bring in required sports physical forms at the scheduled appointment time.
Work Physical- required for some employment.
Adult Annual Physical- administer appropriate vaccinations and order appropriate testing.
Well Women Examinations- including breast, pelvic examinations and pap smear exams.
Pre-operative physical (surgical clearance examination)- please obtain a request from your surgeon faxed to our office or on a prescription.
